Who Can Make a Referral?
- Referrals can be made by professionals who know the child and family and have identified that the child may benefit from a specialist early years placement. Referrals are accepted from: • Health colleagues • Social Care • Early Years Providers • Family Hubs • Talking Together • Portage
- We recognise that parents and carers know their child best. Families are central to the referral, assessment and transition process, and their views, knowledge and priorities will inform the support offered.
- The Phoenix Nursery works collaboratively with families and professionals to develop a shared understanding of each child's strengths, interests, communication, sensory profile, regulation needs and individual learning pathway.
